Additionally, the services we offer are not provided exclusively to seniors. Whether you’re disabled, an injured athlete, or anyone recovering from an operation or illness, LifeSpring pledges to compassionately care and support those individuals in the comfort of their own homes.

Medicare covers 100% of the cost of home health care.

HOME CARE TEAM

RN Case Manager: This licensed and trained professional coordinates and manages the patient’s care, evaluates their condition and needs, and implements the Physician’s treatment plan.

Skilled Nurse: Assists the RN Case Manager in carrying out the treatment plan, providing care, education, and monitoring the condition of the patient.

Rehabilitation: Including Physical, Occupational, and Speech therapists who work closely with caregivers and patients to achieve a higher level of independence.

Medical Social Worker: Assists patients and families with accessing financial resources, facilitates emotional and community support, provides counseling, and helps with transitioning to a new environment such as assisted living or a move into a relative’s home.

Certified Home Health Aide: Assists the patient with bathing, grooming, ambulation and the activities of daily living.
