Position:

Status:

Degree of Travel:

  • Daily home visits & weekly office meetings.

Salary Range:

  • $50,000 - $75,000                    

Description:

  • Provides and directs the provision of home nursing care, based on agency policies and procedures. Evaluates and regularly reevaluates the nursing needs of the client; initiates, develops, implements and makes necessary revisions to the client’s plan of care.

Home Health Occupational Therapist

Location:

• Central Oklahoma City
• Norman

Salary:
$60k - $75k
Your annual salary is based on the per visit rate schedule.


Responsibilities and Duties:

• Provides skilled occupational therapy in accordance with the physician’s plan of care. Accepts only clients referred by the client care manager or designee. Demonstrates competency in skills required by the agency on at least a biannual basis.
• Assists the physician in the evaluation and assessment of the client’s level of functioning by applying diagnostic and prognostic procedures and tests and assists in the development and revision of the plan of care.
• Supervises occupational therapy assistants assigned to license in accordance with licensing board requirements, payer requirements, and agency policies.
• Treats the client through the use of therapeutic activities designed to restore function and self-care activities for the purpose of improving function.
• Observes records and reports the client’s reaction to treatment and any changes in the client’s condition to the physician and/or client care manager and case manager.
• Instructs and advises client, family and other health team personnel in the exercise program designed to increase the client’s strength and control.
• Cooperates with appropriate staff to provide staff education when requested and arranged by the client care manager.
• Demonstrates and teaches alternate techniques to complete activities of daily living, proper transfers and positioning.
• Orders and/or fabricates adaptive/assistive devices and equipment.
• Evaluates, and regularly reevaluates the therapy needs of the client; develops implements and revises the client’s plan of care. Assesses the client’s continual care needs. Ensures that the plan of care addresses all problems identified in the assessment or documents rationale for not doing so.
• Initiates diagnostic, preventive and rehabilitative procedures as appropriate to the client’s care and safety. Makes referrals to other disciplines as indicated by the needs of the client or documents rationale for not doing so.
• Observes signs and symptoms and reports changes in the client’s condition to the physician and/or other appropriate health professionals as often as needed.
• Teaches, supervises and counsels the client and family regarding home rehabilitation procedures and other care needs as appropriate to the client’s condition. Utilizes agency educational material as appropriate.
• Coordinates the total plan of care and maintains continuity of client care by liaising with other health professionals assigned to the same clients. Attends client care conferences. Initiates client care conferences for complex and/or multidisciplinary clients whenever needed to ensure good coordination of care.
• Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies. Submits accurate documentation within 24 hours of visit.
• Participates in the agency’s quality improvement program.
• Keeps abreast of occupational therapy trends and knowledge.
• Participates in in-service programs.

Experience Requirements:
• Registered by the American Occupational Therapy Association or successfully completed the educational requirements of occupational therapy and licensed in the state.
• Minimum of one year of occupational therapy experience.
• Knowledge and Abilities:
o Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population and to a lesser degree the pediatric and adult populations.
o  Demonstrated knowledge of the principles of growth and development over the life span.
o  Able to assess data reflecting the client’s status and interpret the appropriate information needed to identify each client’s requirements relative to their age-specific needs.

Hospice Community Marketing Liaison

Responsibilities and Duties:

  • Responsible for developing and implementing programs to raise and establish community connections by conducting personalized visitations to all primary referral sources.
  • Responsible for generating business from assigned territory referral sources to meet territory and agency revenue and census goals.
  • Coordinates and participates in all marketing projects and campaigns.
  • The Marketing Liaison will build and strengthen business relationships within the community serving as a marketing / sales liaison. This includes serving on community boards, visiting physicians, and attending community events, health fairs, business development activities and sponsored events.
  • Works with internal clinical and administrative team to ensure growth opportunities and achieve monthly goals.
  • Community education to heighten community awareness regarding new and existing physicians and services.
  • Must ensure your work aligns with the overall marketing goals and philosophies of the company. Reporting on progress of referral sources is key. Ability to arrange and lead public relations campaigns and other awareness efforts.
  • Identifies new referral sources by utilizing available corporate resources and his/her community contacts.
  • Works with Corporate and Administration to implement and evaluate territory marketing plans to achieve goals and implement strategies and actions.
  • Maintains key business accounts and builds relationships with prospective accounts. Develops and maintains working relationships with physicians, nursing staff and key administrative personnel in the community.
  • Works with referral sources to improve understanding of our services.
  • Maintains a data base for all referral sources.
  • Facilitate problem resolution with referral sources.

Experience Requirements:

  • Prior marketing/sales experience for Hospice

  • Business development experience

  • Strong technical skills

  • Experience completing legal’s and informational content

  • Strong professional work ethic and team player

  • Organizational, multi-tasking, and prioritizing skills

  • Must be proficient in accounting principles/procedures with strong proficiency in Excel.

  • Must have strong customer service orientation. Must have excellent verbal and written communication skills and be able to communicate effectively across all levels of the organization

  • A Bachelors Degree is preferred

Establishes Plan of Care and coordinates the services needed to address complex chronic illness, disability, or developemental disability.  Directs the provision of home nursing care, therapy, supportive and personal care services, as well as ancillary services and resources, based on agency policies and procedures.  Provides nursing care through the competent application of the nursing process.  Ensures that patients have access to services and opportunities that enhance their development, autonomy, and independence, productivity, well-being and capacity for social interaction whenever possible.  This is a full time position.

Qualifications:

A.  RN with a Master's degree with a specialty in psychiatric or mental health nursing and licensed in the state of Oklahoma. Must have RN nursing experience in an acute treatment.

B. RN with Bachelor's degree and licensed in the state of Oklahoma. Must have one year of recent nursing experience in an acutre treatment unit.

C. RN with a Diploma or Associate degree and licensed in the state of Oklahoma. Must have two years of recent nursing experience in an acute treatment unit.
